Regarding med form for Boy Scout Registration

Dear scout parents,

Regarding the required BSA medical forms for this year’s registration, it is important that I have the med forms of all active scouts on file and up to date, but not all of you need to send me a new or updated form, so please read the following carefully to see if you need to.

You do not need to send me a form if all of the following 3 conditions are true:

(1) I have an up-to-date BSA med form of your scout, such as all those who have gone to summer camp, Canoe trip, or Seabase.  Or you might have given me the form within the last few months.  If you are not sure, send me an email to check.

(2) The last physical checkup must be within 12 months; otherwise, it is out of date.  And it must be on the 2009 BSA med form (2008 is ok).

(3) There is no change in the medical history and conditions.

If you think your med form on file is ok, please make a note on the registration form to let Liz Foley know.  If any one of the conditions is not satisfied, you should send me a new or updated med form, together with the current health form with a physical checkup date of less than 12 months from today, plus a copy of the insurance card, front and back.  It is always a good idea to send me a new health form as soon as your scout has a checkup, any time of the year.  Also, the above applies to adults who plan to register and especially to those adult leaders who plan to participate any scout outings.

You can access the fillable 2009 BSA med form on the troop website, and a sample form is also available. Send it to me by email (pdf format) or by regular mail (address in scout directory).  Keep a copy for yourself.
